Jerry M. Lewellyn, 69 years of age, passed away in Las Vegas, Nevada on September 3, 2011.

He leaves behind four children, Thomas Lewellyn, Jayne Tooley, Carol Faalevao and Ellen Lewellyn all of California and three brothers, Larry Lewellyn, Jackie Terrell and Dr. John Terrell and eight grandchildren Brandon Meier, Sara Perez, Shelby Babcock, Megan Tooley, Wyatt Toolry, Savannah Faalevao, Andrew Faalevao and James Lewellyn.
